Add canned recipe for Putty as telnet handler

Putty can handle telnet URLs natively. Add a canned recipe to configure it to actually do so. And add one to get the old Windows internal handling back. :-)

Use Putty's -ssh parameter in SSH handler

The canned recipe for Putty as SSH handler depends on SSH to be configured as standard protocol. Remove this dependency by adding the Putty parameter -ssh to the protocol handler command.
